If you have lost your British passport, you must also complete Form LS01.
Photographs Two (2) recent 2" x 2" OR 45 (mm) high x 35 (mm) wide full faced color photographs with a light, plain background and taken less than 6 months ago. The face must be square to the camera with the mouth closed (teeth cannot be shown). Do NOT smile in the photographs and hair can NOT cover the face. If you wear glasses, please remove them beforehand to avoid the possibility of rejections, due to glare and reflection on your glasses.
On the back of one (1) of the photographs must be signed by another person who has known you for more than two (2) years and he/she must have a job. This person must also complete Section 9 on application form C1/C2. The person can NOT be a relative of the applicant or a notary. Born before 1 January 1983 in the UK If you were born before 1 January 1983 in the UK, you must provide:

Born before 1 January 1983 outside the UK If you were born before 1 January 1983 outside the UK, you must provide:
Your ORIGINAL full (shows parents details) foreign birth certificate AND
Your father's UK ORIGINAL birth certificate and marriage certificate OR
Your ORIGINAL UK citizenship/registration certificate
Born after 1 January 1983 in the UK If you were born after 1 January 1983 in the UK, you must provide:
Your ORIGINAL full long form UK birth certificate AND
Your mother's ORIGINAL full UK birth certificate OR
Your father's ORIGINAL full UK birth certificate AND
Born after 1 January 1983 outside the UK If you were born after 1 January 1983 outside the UK, you must provide:
Your ORIGINAL full foreign birth certificate AND
Your mother's ORIGINAL full UK birth certificate OR
Your father's ORIGINAL full UK birth certificate AND
Your parents' ORIGINAL marriage certificate AND

Children born after 1 July 2007 outside the UK If the passport is for a child born after 1 July 2007 outside the UK, you must provide the child's ORIGINALS:
Full foreign birth certificate AND
Mother's full UK birth certificate OR
Father's full UK birth certificate AND
Parents' marriage certificate. Note: Foreign Birth Certificate (No. 1 Above) MUST be issued within 3 months from the time of birth. If not available, please supply hospital records of the applicant's birth.
A professional letter, must be fully typed and signed on a business letterhead by the person who countersigned your photo. The letter must state how long the person has known you, the relationship between you and that person, and also state in writing ("I certify that this is a true likeness of "Applicant's Name").
A Copy of Two (2) valid photo IDs (i.e. Copies of Driver's License AND Permanent Resident Card, OR US or Canadian Passport). If you are unable to provide One (1) of the photo IDs above, you MUST provide another (second) Professional Letter as indicated on No. 4 Above. However, this second Professional Letter must be provided from a different person than No. 4. You also need to provide 1 more photograph (in addition to the required Two photographs).
Please provide ALL of the ORIGINAL marriage certificate, AND/OR divorce decree, OR court order if your current name is different than the one on your birth certificate.
Please note that there is no expedited processing for New UK Passport applicants. If you are traveling less than 4 weeks after we process your application, we suggest you to use your other passport (i.e. US or Canada passport) to travel while waiting for your UK passport. We will try our best to meet your travel deadline in cases if you need the British passport to travel.
