Replacement of a Lost or Stolen Passport for a New Passport
Please follow the procedure accordingly to avoid processing delay. An incomplete, incorrectly completed application or failure to follow the procedure will delay the issuance of the passport.
You may use Application for U.S. Passport (Form DS-11) if you are:
Have had a passport, but was lost or stolen.
Have a passport, but is now damaged or mutilated.
Applicant(s) must make a personal apperance and have the following documents executed at an authorized Acceptance facility. Click here to find the nearest Acceptance Agent.
Note: Please advise the acceptance agent that you are using a "hand carry" service to submit your passport application. The agent will verify your identity and witness ("Execute") your application, and upon your request, have the agent execute the documents inside a sealed envelope. Do Not leave the official sealed envelope with the agent and do not open the official sealed envelope, only passport personnel are authorized to open the sealed envelope.
Please present and have the following documents witnessed and sealed by the Acceptance Agent executing your application:
Application Form DS-11. This form MUST be filled and printed out online. Please click here to fill out the form online and print it out. Do NOT sign the application form until a Passport Acceptance agent instructs you to do so. Please print 2 (Two) copies of the application form DS-11 after you filled it out. 1 Copy goes inside of the sealed envelope and the extra copy is for our record.
Two (2) passport type photographs. They must be recent, identical, and 2x2 inches.
A proof of U.S. citizenship. For proof of citizenship, you need to submit one of the following:
A previous U.S. passport (damaged/mutilated); OR
A certified birth certificate issued by state, county or city with raised seal (if born in the U.S.); OR
Certificate of Citizenship (FS-240 or DS-1350); OR
Certificate of Naturalization issued by the Immigration and Naturalization Service.
A statement regarding Lost, Stolen or Damaged Passport (Form DS-64), must be fully completed and signed. This form is needed to prove to the U.S. State Department that you have lost or had your passport stolen and the form must be completed in detail.
Travel itinerary - Copy of a plane ticket, or a travel itinerary with the applicant's name showing the dates and flight information for a departure date within 1-2 weeks in case if you need visas beforehand. Travel itinerary is also needed if you need rush processing.
Include a $170.00 government fee, payable with a money order to U.S. Department of State. Please indicate your Date of Birth in the memo field on your money order. Please note that you will also have to pay $25.00 execution fee to the Acceptance Agent at the Post Office.

Important Note: If you require a Rush processing, you MUST send your documents to us by FedEx First Overnight and call us with your FedEx tracking number. Fail to do so will delay your passport processing.
