U.S. Passport Renewal

Applicants Age 16 and Over 

Please follow the procedure accordingly to avoid processing delay. An incomplete, incorrectly completed application or failure to follow the procedure will delay the issuance of the passport.

You may use Application for U.S. Passport Renewal (Form DS-82) if you are:

• Already have a passport that is not damaged; AND
• Received it within the past 15 years; AND
• Recent passport was valid for 10 years; AND 
• Were over age 16 when it was issued; AND
• Still have the same name as in the passport (or i can legally document my name change by marriage or court order.

1. Application Form DS-82. This form MUST be filled, printed out online and signed.

2. Two (2) passport type photographs.
   They must be recent (taken less than 6 months), identical, and 2x2 inches.
   
   
3. Your most recent passport as described above.
   If your name has changed since your passport was issued, please enclose one of the following:
   • A certified copy of the legal document specifying your name change, such as marriage certificate, divorce decree, adoption decree, or court order.

6. Travel itinerary.
   Copy of a plane ticket, or a travel itinerary with the applicant's name showing the dates and flight information for a departure date within 1-2 weeks in case if you need visas beforehand. Travel itinerary is also needed if you need rush processing.
   
   
7. Payments: (Please write 2 separate money orders: one for U.S. Dept. of State for government fee, the second money order is to E&G Visa Services for our service fee and return mailing fee)

• Government Fee: $170.00, payable with a money order to U.S. Department of State. Please indicate your Date of Birth in the memo field.

  $95.00 per person - 7 to 10 business days processing.
