U.S. SECOND LIMITED PASSPORT

Please follow the procedure accordingly to avoid processing delay. An incomplete, incorrectly completed application or failure to follow the procedure will delay the issuance of the passport.

You may use Application for U.S. Second Limited Passport (Form DS-82) if you are:

• Can submit your most recent U.S. passport; AND
• Were at least 16 years old when your most recent U.S. passport was issued; AND
• Were issued your most recent U.S. passport less than 8 years ago; AND
• Are using the same name on your most recent passport; AND
• Can show a valid reason for needing a second limited validity passport to the U.S. Department of State.

1. Your original current passport with at least two years of validity remaining and passport must be signed.
   
   
2. Application Form DS-82. This form MUST be filled, printed out online and signed.

3. Two (2) passport type photographs. They must be recent, identical, and 2x2 inches.
   
   
4. Statement requesting a Second Limited Passport, fully completed and signed by applicant.

8. Payments: (Please write 2 separate money orders: one money order for the U.S. Dept. of State for government fee, the second money order is to E&G Visa Services for our service fee and return mailing fee)

• Government Fee: $170.00, payable with a money order to U.S. Department of State. Please indicate your Date of Birth in the memo field.
